A daycare sign-in sheet is a form that parents fill out when dropping their children off at daycare, preschool, afterschool, or other child care programs. They sign it again when they pick their children up. This provides an official record of when the child was in the care program.
Childcare providers must follow the policies and procedures of their licensing agency to ensure child safety. As part of these regulations, many states require the use of daycare sign-in sheets to help track attendance. If you aren’t sure about the laws in your state, follow this link to review them. Some states, like Wisconsin, even require family childcare centers that accept a childcare subsidy to use a specific form. But, most centers are expected to create their own attendance log.
Even if such records are not mandatory for your childcare licensing, there are still benefits to using attendance sheets. They help the staff know exactly which children are on-site. This makes it easier for you to ensure everyone is accounted for and helps with some day-to-day logistics, such as meals.
Also, gathering this information helps you quickly calculate the payment due by each family. And by requiring parents to sign next to their pickup time, they are forced to acknowledge if they are late. This makes it simpler to enforce your late policies and collect any associated fees.
To help centers remain compliant, we offer a free printable template with three different sign-in sheets.
This Excel daycare sign-in sheet template contains three different attendance tracking worksheets. Here is a list of what is included.
Generic In Out Color Table Daycare Sign-In Sheet
This colorful sign-in sheet is dated and designed to be printed daily. As parents drop their children off in the morning, they fill in the form. Then the next parent signs in below them.
With this worksheet, teachers can see at a glance which children were in their care on a particular day.
However, it does not have a spot for a parent signature, so if your regulations require that, this version may not be a good fit, unless you change one of the existing columns to “signature.”
Daily In Out Month Portrait Daycare Sign-In Sheet
Some daycare centers prefer to keep individual attendance records. This sample form does that. Each month, you print out a copy of the attendance tracker for each child in your care.
As the parents arrive, they locate their child’s form and sign in. When they leave, they sign them out.
With portrait orientation, this free printable worksheet easily fits into a three-ring binder or on a clipboard.
Daily In Out Month Landscape Daycare Sign-In Sheet
Created in landscape orientation, this daily child’s sign-in and sign-out sheet can be used for all the children in your care. Since there is not a date on the form, you can use the same sheet over multiple days.
Parents will sign this version of the form twice each day, once when signing their children into daycare, and once when picking them up.
How to Customize and Use the Template
Please follow instructions below to learn how to customize our free Daycare sign-in sheet.
Generic In Out Color Table
You can customize this sign-in sheet to match your daycare’s branding.
Click on the name “Morning Glory Daycare” to enter in the name of your childcare center. If you need a space for a parent or guardian signature, you can change the Notes column to read Signature. If you want to collect some other information (allergies, medication administration, etc.) you can change this column to fit your needs.
Here are some quick tutorials on how to change the image and colors to customize this daycare sign-in sheet:
The column “Notes” can be changed to include other information you want to collect – for example, emergency contact, special meal information, etc.
When you finish customizing your childcare center form, it’s ready for use. Just make sure you change the date before printing!
You can print out a new form each day, or you can print them in advance for the whole week. But if you do this, remember to verify that you use the correct date!
When parents arrive, they record the time in and other requested information. Then when they come to collect their children, they note the time out.
Daily In Out Month Portrait
There are some benefits to having individual sign-in forms for each child in your care. Instead of having a bottleneck form around the sign-in sheet on hectic mornings, you can make the daycare sign-in process more efficient with individual sheets.
To customize this form, you can change the generic name to reflect the name of your care program. You can also follow the tutorials above to change the colors of any cells.
You will need to print out a copy of this attendance tracker for each child in your care. You can either type in the child’s name, the month, and the year, or you can manually write it in after you print them.
You can store these on clipboards, in three-ring binders, or in any other system you choose. Each morning, the parents will locate their child’s form and fill in the arrival time. When they pick their children up, they write their departure time and sign the document.
Daily In Out Month Landscape
An undated form, this daily attendance log is ready to print after you customize it.
You can change the header to reflect your company’s name. You can also follow the tutorials linked above to change the colors.
Print out your sign-in sheet and keep it in an easy to access location. When parents drop their children off, they record the date, their child’s name, and the time in. They also sign.
When picking a child up, the parent simply records the time out and signs again.
Keep Your Daycare Records Organized
These daycare sign in sheets can help you manage attendance tracking and keep your daycare records organized. By filing each sheet as it gets completed, you can easily go back and review them. You will be able to see which children were in attendance each day that your center is open.
So pick which one of these child care forms makes the most sense for your daycare’s policies, then download and customize it. Then print it out and place it in an easy to access location.
